The Talc Files: The Powder We Trusted, and the Question It Never Quite Answered

Siobhan Ryan grew up watching her mother reach for the same white bottle every day. Later she used it on herself, and then on her own children, because it smelled clean and felt soft and every advert she had ever seen promised it was the gentlest thing a parent could put on a baby. “It smelt nice and was soft and lovely,” she told the BBC, and for years she believed she was doing right by the people she loved most.

She was diagnosed with stage four ovarian cancer a little over eighteen months before she told the BBC her story. She is now one of more than three thousand people suing Johnson & Johnson in the United Kingdom, in a claim filed by the law firm KP Law that could run into the hundreds of millions of pounds. She believes the powder helped cause her disease.

Whether anyone can actually prove that is where this story gets complicated, and it is worth being honest about the real question before going any further. It is not simply whether talcum powder causes cancer. It is how one of the most ordinary products on earth, sold for over a century on a promise of pure gentleness, ended up at the centre of a fifty-year scientific argument, a legal fight worth tens of billions of dollars, and, as of this year, a retraction inside one of medicine’s most trusted journals. That last part turns out to matter more than it sounds. Getting there takes some patience, so this is worth doing properly, in order.

In the rock itself

The story does not begin in a courtroom. It begins underground, in the rock itself. Talc and asbestos are geological neighbours, minerals that often form under the same conditions in the same deposits, which means talc ore has always carried one unavoidable risk: that asbestos fibres travel with it unless the ore is carefully screened. Johnson & Johnson has always said its cosmetic talc was pure. Internal company documents, cited in the BBC’s review of the 2025 UK court filing, complicate that. A 1969 memo referred to test results showing trace asbestos in some talc samples. A 1973 memo discussed a method for removing asbestos fibres and weighed whether patenting it might tip off the world to a problem the company preferred kept quiet. Kenvue, which now defends these documents in court and responded directly to the BBC’s questions, says the record has been misread and shows no contamination of products actually sold to consumers.

1977: an unsigned all-clear

Into that quiet argument, in 1977, one of the most respected medical journals in the world published an unsigned commentary concluding there was no good reason to think cosmetic talc caused cancer, and no need for tighter asbestos testing. It read, at the time, as independent and authoritative, exactly the kind of disinterested word regulators and the public needed at a moment when new rules were being weighed. Nobody who read it had any reason to wonder who had actually written it, and, as The Lancet’s own retraction notice would later confirm, that question would sit unanswered for forty-nine years.

The harder question

While that commentary quietly did its work, a separate and genuinely harder scientific question was opening up: not about asbestos, but about pure talc itself, used exactly as intended. In 1982, a Harvard researcher found that women with ovarian cancer were somewhat more likely than healthy women to recall a history of genital talc use, a finding summarised in later systematic reviews, including a 2024 review in Critical Reviews in Toxicology and a 2023 review in Frontiers in Toxicology, both of which trace the same pattern across dozens of subsequent studies. But those same reviews note that every large study which instead followed healthy women forward in time, watching what happened rather than asking people to remember, found nothing. That split, real in memory, absent in observation, has never resolved. The most rigorous recent attempt to quantify it, a 2026 meta-analysis in the journal Cancers, confirmed the pattern rather than settling it: a modest positive association in case-control studies that held up even when any single study was removed from the analysis and rose with longer and more frequent use, against no association at all in the pooled cohort data. Its own authors disclose that they work as contractors for a litigation consulting firm and that the analysis began as litigation consulting work, and, to their credit, they say plainly that the finding does not establish causation. In 2010 the International Agency for Research on Cancer placed talc in a cautious middle category, possibly carcinogenic, the same tier as aloe vera extract. Not an alarm. Not a clearance either.

Mixed finding

The lawsuits arrive

The lawsuits arrived first over the weaker of the two questions. A February 2016 St Louis verdict, reported at the time by Stuff, was awarded to the family of Jackie Fox, an Alabama woman who had used the powder daily for decades and died of ovarian cancer before the case was decided; the jury found the company liable for roughly seventy-two million US dollars. But the case that reshaped the whole fight came two years later. As detailed in a 2019 review for physicians published in Missouri Medicine, the 2018 Ingham verdict argued something narrower and scientifically far sturdier: not that talc caused cancer, but that this talc, contaminated with asbestos, did. Twenty-two women were awarded over four and a half billion dollars combined. From that point on, the strongest cases in the whole litigation were rarely about talc on its own. They were about what might have been hiding inside it, since asbestos is an established human carcinogen with a clear, well-understood link to a rare cancer called mesothelioma, in a way pure talc and ovarian cancer had never managed to demonstrate.

In 2019 the FDA’s own testing found asbestos in one lot of the product, the first time a regulator’s testing had confirmed what the company had spent decades denying was possible. The powder came off American shelves the following year, and off shelves everywhere else by 2023, as the BBC and NZ Herald both reported. Facing tens of thousands of lawsuits, Johnson & Johnson then tried, according to CNBC’s coverage of the May 2024 proposal, to resolve everything at once through a bankruptcy manoeuvre, offering 6.475 billion dollars over twenty-five years contingent on claimant approval. That failed, as did an earlier 2023 attempt and a final one near eight billion dollars rejected by a federal court in the spring of 2025, a sequence later recapped by the injury-law website Asbestos.com. After three defeats, the company abandoned the strategy to fight every case individually.

Who paid for the reassurance

It was around this same time that the scientific literature itself pulled in two different directions. In 2023, a review in Frontiers in Toxicology concluded there was no real evidence linking talc to ovarian cancer. In 2024, a second review, in Critical Reviews in Toxicology, reached the same conclusion. Each paper’s own published conflict-of-interest section says who was behind it. The 2023 review was funded by a group called the Center for Truth in Science, and several of its authors had worked as paid defence witnesses in talc trials. The 2024 review was funded by the Essential Minerals Association, a mining trade group, and its authors say plainly they prepared it for submission to the International Agency for Research on Cancer, the same UN body then finishing its own review of talc.

That review went the other way. In 2024, reported in New Zealand by RNZ, the IARC reclassified talc from possibly to probably carcinogenic, the opposite direction from the two papers written for its consideration. None of this proves the funded reviews were wrong. The large, genuinely independent cohort studies really have kept coming back null, and that particular finding does not depend on who is paying for it. What it does mean is that a meaningful share of the calm, reassuring science in circulation at that moment had been funded by one side of an argument the world’s own cancer agency was, at the very same time, leaning away from. That fact would matter again eighteen months later.

2025: the verdicts stack up

The verdicts that followed in 2025 were mostly, again, about asbestos and mesothelioma rather than ordinary talc use. A tally kept by Rheingold Law, a firm that represents talc plaintiffs and whose own blog post is the source for these figures, put a Baltimore award at 1.5 billion dollars to Cherie Craft, who developed peritoneal mesothelioma after a lifetime of using the powder, the largest single-plaintiff talc verdict on record, and a Los Angeles award at 966 million to the family of Mae Moore, who had died of the same disease, after jurors found the company acted with malice. The Guardian’s own reporting covered one of the rarer ovarian-cancer wins that year in detail: a Los Angeles jury awarded forty million dollars in December to two women, Monica Kent and Deborah Schultz, diagnosed years apart and each treated with major surgery and repeated rounds of chemotherapy, after their attorney told the court the company had known since the 1960s that its product could cause cancer, a claim J&J’s litigation counsel dismissed to the same reporters as unsupported by any major health authority. Across the Atlantic, the charity Ovarian Cancer Action looked at the same body of evidence and published calmer advice, quoting Imperial College oncologist Professor Iain McNeish on a quarter-million-woman study that found no statistically significant link at all, and telling women who had used the powder for decades not to panic. Both things were true simultaneously, because they were answers to two different questions that had been sitting inside the same word for fifty years.

2026: it all moves at once

Then, in early 2026, everything moved at once. In January, according to Rheingold Law’s litigation update, a federal judge, Freda Wolfson, cleared plaintiffs’ scientific experts to testify at trial after a six-hundred-page review of their methods. In February, Asbestos.com reported, a Philadelphia jury found the company liable in the death of Gayle Emerson, who had used its powder for more than four decades. And on 25 March, The Lancet issued its retraction, a decision covered in detail by Retraction Watch and by Columbia University’s Mailman School of Public Health, whose own historians, Gerald Markowitz and David Rosner, had finally answered the question nobody thought to ask about that 1977 commentary: who wrote it. The author was Francis J. C. Roe, a well-known cancer researcher who had also been a paid consultant to Johnson & Johnson, and who, the historians showed, had shared drafts of the piece with the company and revised it on their feedback before it ran, none of which had ever been disclosed. The journal’s editors called it a clear breach of publishing ethics.

For forty-nine years, an anonymous, apparently neutral voice inside one of medicine’s most trusted journals had been quietly working for one side the entire time.

It did not settle anything. Within days, a California court threw out the 950 million dollar punitive portion of that Los Angeles mesothelioma award, as the industry outlet Personal Care Insights reported. Within months, RNZ, carrying a Reuters report, described a Los Angeles jury siding with Johnson & Johnson entirely in a separate case brought by the families of three women, the same week the company also won a trial in Oklahoma, while Asbestos.com reported that a different Los Angeles jury awarded 32 million dollars to the family of Maria Lozano, who had used the powder on herself and her children for fifty years before dying of mesothelioma, a case her own children carried forward after her death, after her lawyers produced decades of internal testing the company had never shared. Roughly sixty thousand claims remain open.

What the retraction does, and does not, do

None of it tells Siobhan Ryan what actually caused her cancer. Since her diagnosis she has been through three rounds of chemotherapy and a bout of sepsis severe enough that she nearly did not survive it. For a while the treatment seemed to be holding the disease back. Then, a few months ago, she found another lump, and her surgeons told her the cancer is no longer operable. Whatever the studies, the reviews and the verdicts eventually settle, none of it will arrive in time to change what she is now facing, and that is the honest place to end.

Where talc was contaminated with asbestos, the documented history of contamination and non-disclosure is genuinely damning, and it is what nearly every large verdict since 2018 has actually been about. Where the question is pure, uncontaminated talc and ovarian cancer on its own, the picture remains what the 1982 study first suggested and every review since has confirmed: a real signal in memory, nothing in direct observation, and no proven mechanism joining the two. Fifty years, one retracted commentary, and a stack of funded reviews later, that is still the honest answer. The one thing this whole history does make plain is which question to ask first when someone hands you a reassuring study, or, for that matter, a damning verdict tally. Not what it concluded. Who paid for it, and who is telling you about it.
